Maintenance Contract
Between the undersigned
The Maintenance Provider: Maintenance provider name, located at Maintenance provider address, SIRET Maintenance provider SIRET.

Article 1 – Purpose
The purpose of this contract is the maintenance of: Maintenance subject.
Article 2 – Nature of interventions
The contract covers the following interventions:
- Preventive maintenance: Preventive maintenance frequency
- Corrective maintenance: intervention within Corrective intervention time after reporting
Article 3 – Duration and renewal
The contract takes effect on Start date for a period of Contract duration. It is renewable by tacit renewal unless terminated with Termination notice period notice.
Article 4 – Pricing
The annual maintenance fee is set at Annual amount excl. tax (EUR) EUR excl. tax. Spare parts are Spare parts included?.
Article 5 – Exclusions
The following are excluded from the maintenance warranty: Exclusions.
Article 6 – Liability
The Maintenance Provider's liability is limited to the annual contract amount. The Provider shall not be held liable for indirect damages.
